Prescription support for natural tear production
Tryptyr (acoltremon ophthalmic solution 0.003%) is an FDA-approved prescription eye drop for the signs and symptoms of dry eye disease. It stimulates tear production through sensory receptors on the ocular surface. At EnVision Dry Eye Center, it is considered as part of a treatment plan based on your examination findings.
The surface of the eye contains cold sensitive receptors called TRPM8 receptors. These receptors participate in signaling that helps regulate natural tearing. Acoltremon activates this pathway to increase tear production. This approach differs from adding artificial tears to the eye or treating inflammation alone.
Dry eye can involve both an inadequate supply of tears and poor tear stability. Increasing tear production may address one part of the problem, while eyelid disease, excessive evaporation, or inflammation may need additional care.
Dr. Jason Besecker reviews your symptoms, current treatments, and ocular surface findings before recommending a prescription. The evaluation considers tear quantity alongside tear quality, eyelid health, and surface damage. A low tear measurement does not by itself determine the entire treatment plan.

In the randomized COMET-2 and COMET-3 trials, more patients using acoltremon achieved a meaningful increase in Schirmer tear test measurements than patients using vehicle drops. Increased tear production was observed early in treatment and was maintained across the 90-day studies.
A tear test improvement is an objective measurement; it does not mean every patient becomes symptom free. We assess your comfort, daily function, and examination findings together when deciding whether treatment is helping.
The labeled dose is one drop in each eye twice daily, approximately 12 hours apart. Your prescriber will discuss follow-up and ongoing treatment.
Instillation site pain, including burning or stinging, was reported in 50% of patients in controlled studies. Contact the clinic for troublesome or persistent irritation, worsening redness, pain, or vision changes. Tell your prescriber about pregnancy or breastfeeding; safety and effectiveness below age 18 have not been established.
Follow the package storage instructions carefully. After opening the carton, room-temperature storage is limited to 30 days. The complete prescribing information and instructions for use are linked below.
Bring a list of your prescription drops, artificial tears, and home treatments to your visit. Our team can help organize the schedule and identify which findings each treatment is intended to address. Do not stop another prescribed treatment without discussing the change with your clinician.
