Non-Invasive Tear Breakup Time

Measuring how well your tears hold together between blinks

What is non-invasive tear breakup time?

Non-invasive tear breakup time, often called NIBUT, measures how long the tear film remains stable after a blink. A smooth, stable tear film supports clear vision and helps protect the front surface of the eye.

This test examines the tear film without adding diagnostic dye for the measurement. It provides a view of tear behavior that complements the other parts of a dry eye evaluation.

Why does tear stability matter?

If the tear film breaks up quickly, the surface may become uneven between blinks. This can contribute to burning, irritation, or vision that briefly clears after blinking and then becomes blurry again.

A short breakup time indicates instability, but it does not identify a single cause. Oil gland dysfunction, tear component deficiencies, blinking patterns, and ocular surface changes can all affect the result.

What happens during the test?

You will look toward an imaging instrument, blink as directed, and hold your eyes open briefly while measurements are taken. The instrument assesses changes in the reflected image or tear film pattern.

Keeping the eye open can be uncomfortable when tears are unstable. Let the team know if you need to blink or pause. Measurements may be repeated because tear behavior can vary from one blink to another.

How are the results interpreted?

Dr. Jason Besecker considers breakup time together with your symptoms, eyelid examination, meibography, tear osmolarity, and surface staining. Results from different instruments or techniques are not always directly interchangeable.

A measurement is one part of the assessment. A relatively stable reading on one occasion does not explain every source of discomfort, and an unstable reading alone does not determine which treatment you need.

How can testing help guide care?

When instability is identified, the next step is to look for its contributing factors. Your plan may address gland function, blinking, inflammation, lubrication, or other findings rather than focusing only on a number.

Repeat measurements may help assess tear stability during follow up when appropriate. Changes are interpreted alongside your comfort, daily function, and the rest of the examination.
